sociated with albuminuria in type 2 diabetic patients (4). To the best of our knowledge, however, it is not clear whether serum homocysteine is associated with TNF receptor in type 2 diabetic patients. The aim of the present study was therefore to investigate the relationships between serum homocysteine and TNF receptor in patients with type 2 diabetes. Fifty nonobese Japanese type 2 diabetic patients were studied. Their BMI, HbA1c, and serum creatinine were 22.6 0.3 kg/m2 (range 17.6 –26.2), 7.8 0.2% (5.5–12.3), and 0.70 0.02 mg/dl (0.46–0.98), respectively. They had not been treated with insulin or any medications known to alter homocysteine level. In conjunction with homocysteine, systolic and diastolic blood pressure, HbA1c, glucose, lipids, serum creatinine, TNF, sTNF-R1, and sTNF-R2 were measured after an overnight fast. With univariate analysis, serum homocysteine was positively correlated with age (r 0.361, P 0.012), diabetes duration (r 0.292, P 0.045), serum creatinine (r 0.623, P 0.001), sTNF-R1 (r 0.415, P 0.005), and sTNF-R2 (r 0.371, P 0.01). Other variables including TNF, however, were not associated with homocysteine. Multiple regression analyses showed that serum homocysteine was independently associated with serum creatinine (F 20.1) and sTNF-R1 (F 6.9), which explained 49.3% of the variability of homocysteine. Thus, TNF system activity may be responsible for the evolution of atherosclerosis induced by homocysteine in nonobese Japanese type 2 diabetic patients.

Lactic acidosis and hyperamylasaemia associated with phenformin therapy.
Introduction Lactic acidosis, first described by Clausen (1925), is most frequently seen accompanying hypotension and impaired tissue perfusion, but has been reported in association with many other conditions such as phenformin therapy, non-ketotic diabetic acidosis, ethanol ingestion and glycogen storage disease (Oliva, 1970; Lancet, 1973). Recently, an association was suggested between non-ke...
متن کاملPhenformin and lactic acidosis.
Thirty-eight patients who presented with diabetes and a changed state of consciousness satisfied the criteria for lactic acidosis. Sixteen patients were non-ketotic, and 15 of these were receiving phenformin on admission. In all but one of these 15 patients, however, additional renal or cardiovascular abnormalities, or both, could be identified, which supported a multifactorial aetiology for la...

Case history The patient was a 47-year-old woman who developed diabetes mellitus in 1965. She was initially controlled on chlorpropamide and phenformin. However, her diabetic control became less satisfactory and insulin therapy was substituted in October 1967. Subsequently her diabetes became unstable and phenformin 100-150 mg daily was added in November 1968. Since then her blood glucose level...
متن کاملEpidemiology of adverse drug reactions to phenformin and metformin.
Adverse drug reactions (ADRs) to phenformin and metformin reported to the Swedish Adverse Drug Reaction Committee during 1965--77 were analysed in relation to sales and prescription data. The biguanides accounted for 0.6% of all reported adverse drug reactions but for 6% of the fatal cases (all phenformin). Sixty-four ADRs to phenformin and eight to metformin were classified as causal relation ...
متن کاملMetformin and Lactic Acidosis
Probably the most common mechanism by which metformin elevates blood lactate is by inducing catecholamine release in those who regulate or prescribe it. Caution in restricting the use of metformin for the treatment of type 2 diabetes has been rationalized on the basis of J) its chemical identity as a biguanide, 2) its ability to modify hepatic intermediary metabolism, 3) its administration to i...
